Baptism Certificate

Use this form to request your Baptism Certificate (if baptized at SVdP only) or Confirmation Certificate (if confirmed at SVdP).

Required Documents and Process

• Copy of birth certificate or valid ID that includes full name and date of birth, i.e. passport, driver’s license
• Adults requesting a baptism certificate for their child must submit a copy of his/her ID 
• Email the required document(s) to Kelly Hermann at khermann@svdp-edu.org
  (Note, if certificate is sent DIRECTLY to another parish, the above ID documents are not needed)
• Please allow five business days to process your request
• Submit the form below
